What is the Planning for Healthy Babies® program?

P4HB® provides no cost family planning services to eligible women in Georgia. There are three levels of service:

  • Family planning only
  • Inter-pregnancy care (IPC)  
  • Resource Mother only
  • What’s covered?
    • Annual physical exams including pap smears
    • Contraceptives and multivitamins with folic acid
    • Family planning counseling
    • IPC services including primary care and dental services, substance abuse treatment services, a Resource Mother and a nurse case manager.
    • Resource Mother only services include help with care for your very low birth weight (VLBW) baby. The Resource Mother also helps you take care of your health too
  • Who is eligible?
    • Women ages 18 through 44 who meet monthly family income limits and are able to have a baby
    • Women who do not receive Medicaid or Medicare are eligible for family planning services
    • Women who have given birth to a baby weighing less than 3 pounds, 5 ounces and do not receive Medicaid including Medicare or are losing Medicaid coverage, are eligible for IPC services
    • Women who are receiving Medicaid and have given birth to a baby weighing less than 3 pounds, 5 ounces are only eligible for Resource Mother services

    • Federally Qualified Health Centers
  • Where can I download the form, that my physician is supposed to fill out if I answered yes to the question on having delivered a baby weighing less than 1500 grams (3.3lbs)?

  • When can I receive services?

    Your service starts once you have been enrolled in a health plan.

  • How can I enroll in a health plan?

    We will enroll you in a health plan to receive P4HB services. If you do not want the health plan we choose for you, you can change your health plan. You will have 90 days from the start date of your new health plan to change your health plan. Your health plan has doctors and other people who will take care of your health needs to keep you healthy.

    The P4HB Health Plans in Georgia:

  • How do I find a provider?

    You will need to choose a provider in your new health plan. For family planning services, you may choose any provider, even one that is not in your health plan. If you do not choose a provider, your new health plan will choose one for you.

  • What services will I get?

    Women enrolled in Inter-pregnancy care services will get limited primary care, family planning, limited dental services, a resource mother and a nurse case manager, prescription drugs (non-family planning) and substance abuse services.

    Women enrolled in family planning services will only have access to family planning providers. Primary care needs will be provided through referrals to community health centers and rural health centers

  • If I have received a request to send in my citizenship and identity documents to P4HB, how can I return the requested documents or where can I go?

    P4HB applicants and members can upload their documents in the Georgia Gateway system at www.gateway.ga.gov or members can contact one of the approved organizations listed below to return documents for citizenship and/or identity.
